Community Behavioral Health Centers can be highly siloed systems with medical and clinical providers missing opportunities to collaborate, and limited engagement with primary healthcare systems and community stakeholders to address social determinants of health. To be effective in a value-based care environment, organizations must focus on the whole health needs of individuals served and will need to become team based, interactive and integrated.
